We provide services to Nursing homes and long term care facilities. These services include proceeding medication orders and preparing dispill sheets for Nursing Home residents. In addition, the pharmacist has an active role in the Nursing Home and provides in depth Medication Reconciliation. The Pharmacist works together with other healthcare professionals in order to ensure patients receive optimal treatment. During comprehensive medication reviews, medications are reviewed consistently – every three months (TMR’s) to ensure that medications being added, changed or discontinued are carefully evaluated especially across transitions of care.
